Dr. Geoffrey Stiller says his brutal surgeries make patients “happy about who they are.” They’ve also made him millions in Medicaid payments, according to The Daily Wire.

Washington, the northern state that borders Canada, allegedly gave millions of dollars from taxpayers to a controversial surgeon for performing “gender affirming” surgeries on prison inmates, according to The Daily Wire.
According to The Wire, Dr. Geoffrey Stiller, who leads Stiller Aesthetics, a cosmetic surgery practice, has received more than $4.3 million from the Washington State Health Care Authority since July 2019, the state spending database shows.
Stiller offers a range of “transgender” surgical procedures including facial feminization, “non-binary” double mastectomy, and “bottom surgery,” according to the Stiller Aesthetic website.
“Bottom surgery” is a synonym for vaginoplasty surgery, a procedure that entails the removal of male genitalia and frequently uses colon tissue to create a fake vagina, called a neovagina, which needs a lifetime distention to prevent it from narrowing or closing.
According to public records revised by The Daily Wire, they show that Stiller’s clientele included inmates from the Washington Department of Corrections, whose policies support inmates pursuing tax-funded sex-rejection surgeries.
Many sex-rejection interventions are provided to inmates by in-house clinicians, but those considered eligible for sex-change surgery can request for temporary coverage through Washington state’s Medicaid program, Apple Health, which gives coverage for transgender surgeries.
In a 2025 Executive Order, President Donald Trump prohibited the use of federal funds to pay for “any medical procedure, treatment, or drug for the purpose of conforming an inmate’s appearance to that of the opposite sex.”
Still, in February 2025, Washington State led a federal lawsuit by several states against the Trump administration, and received a preliminary injunction against the order that permitted inmates at federal facilities to keep receiving sex-rejection surgeries.
